1. “But Doctor, I’m Only Twenty-Six.”

She sat in our clinic last month pointing at a bulging vein on her right calf, looking more confused than worried. She was twenty-six, fit, did not smoke, had no obvious risk factors. And she could not believe what she was seeing on her own leg.

“This is supposed to happen to my grandmother, not me.”

We hear some version of this every week. Young professionals in their twenties and thirties, seeing varicose veins appear for the first time and assuming there must be a mistake. Maybe it is just a prominent vein. Maybe it is from the gym. Maybe it will go away on its own.

It will not go away on its own. Varicose veins are caused by a structural failure of the valves inside your veins. Valves do not repair themselves. And age, while a factor, is not the main driver. Genetics is.

2. Genetics Does Not Wait for Middle Age

If one of your parents has varicose veins, your risk of developing them is roughly 40 per cent. If both parents have them, it climbs to nearly 90 per cent. That genetic loading does not activate at fifty. It can show up at twenty-two.

What genetics determines is the inherent strength of your vein walls and valves. Some people are born with valves that are structurally weaker, thinner, or more prone to becoming incompetent under normal daily stress. For these individuals, the wear and tear of standing, sitting, and gravity does its damage much earlier than the population average.

If your mother needed vein treatment in her forties, you may need it in your late twenties. Not because you did something wrong. Because you inherited the same vein architecture.

3. The Modern Lifestyle Accelerators

Genetics loads the gun. Lifestyle pulls the trigger. And for the current generation, several lifestyle factors are accelerating varicose vein development in ways previous generations did not face:

  • Desk jobs. Eight to ten hours sitting with legs immobile under a desk. The calf muscle pump, which pushes blood upward, barely activates. Blood pools. Veins stretch. This is the IT corridor problem: Bangalore, Hyderabad, Pune, Gurgaon offices are quietly producing varicose veins in twenty-somethings.
  • Hormonal contraceptives. Oestrogen and progesterone relax vein walls and make valves more susceptible to failure. Young women on long-term hormonal contraception have a measurably higher risk of developing varicose veins earlier.
  • Sedentary lifestyles outside work. After eight hours at a desk, the evening is spent on a couch. Weekends are inactive. The legs rarely get the sustained walking or exercise that keeps the calf pump active and veins healthy.
  • Obesity. Increasing weight puts more pressure on leg veins. India’s young adult obesity rate is rising, and with it, early-onset venous disease.

4. What Young Patients Dismiss (and What It Costs Them Later)

The early signs are easy to dismiss when you are twenty-five. One visible vein. Legs that feel heavy after a long day but feel fine in the morning. Mild aching in the calves that you attribute to the gym or to standing at a concert. Restless legs at night that you Google and get told is “stress.”

These are not random complaints. They are chapter one of a progressive disease. Chronic Venous Insufficiency does not stay at chapter one. Without treatment, it moves through predictable stages: increasing heaviness and swelling, then skin changes around the ankles (darkening, itching, eczema), and eventually, in the worst cases, venous ulcers.

The vein you dismiss at twenty-six does not disappear. It worsens slowly. By thirty-five, there are more veins. By forty, the ankle skin is changing. By fifty, you are managing a chronic wound. That entire trajectory was preventable with a 30-minute procedure at twenty-six.

5. Why Catching It Young Is Actually an Advantage

This is the part young patients need to hear, because it reframes everything.

Treating varicose veins early, when only one or two veins are affected and the disease has not progressed to skin damage, is simpler, faster, and more effective than treating it at an advanced stage.

EVLA at the early stage means treating one faulty trunk vein. Thirty minutes. Local anaesthesia. Walk out the same day. One or two days off routine at most. No skin damage to reverse. No ulcer to manage. Just a clean, quick fix that prevents decades of problems.

Compare that to treating varicose veins at fifty-five after years of skin changes, multiple affected veins, and possibly an active ulcer. The treatment still works, but it is more complex, may require multiple sessions, and the skin damage that has already occurred may only partially reverse.

Early detection is not a burden. It is a gift. A Doppler ultrasound at twenty-six that catches reflux early is the best investment you can make in your leg health for the next thirty years.

6. What to Do If You Are Under 35 and Worried About Your Veins

If you are young and you can see veins that were not there a year ago, or if your legs feel heavier than they should, or if one of your parents has varicose veins, here is what we suggest:

  • Get a Colour Doppler Ultrasound. It takes 15 minutes. It is painless. It shows whether your deeper veins have reflux (backward flow). If they do, we have a clear picture and a clear plan. If they do not, you leave reassured.

  • Do not wait for it to get worse. The disease does not plateau on its own. Valves do not fix themselves. The earlier you treat, the simpler the solution and the better the long-term outcome.
